By sending this form you are aggreeing to the terms and conditions of our affiliate program.

1. All sales generated by you will recv 15% of the total order minus shipping.

2. 20% Discount for personal orders. We will give you a permanent code that you can use at all times.

3. Commisions are paid monthly through paypal or RME

4. You will have a reference code that must be used in the "message box" on all orders in order to recv credit for that order. You also have the option of having your own domain name and pointing it to our site. Domains can be purchased for 7.95 per year at www.hostway.com

5. There is a minimum requirement of one order per month to maintain your affiliate status.

6. For all affiliate referals you will recv $5 after that rep that you referred has generated their first order.

7. You can cancel your affiliate status at any time, we just ask that you let us know. All orders recvd after your resignation will be forfeited.

You can choose to have your payment sent via PayPal or use your
commissions as a store credit to make purchases.

Your account will be activated after it has been approved and you will receive an
email with your affiliate details.

Affiliates are independent distributors and not employee’s of Phat Diva Styles.
 All affiliates are responsible for their own licensing and taxes, based on their state and federal requirements.
